Model Requirements

  • Be at least 18 years of age

  • Complete a medical history and consultation

  • Sign all required treatment consent, model release, and liability forms

  • Allow Sweetwater Aesthetics to photograph and/or record the treatment process

  • Grant permission for photos and videos to be used for educational, marketing, website, and social media purposes

  • Understand that model services are discounted, not complimentary

  • Meet any treatment-specific qualifications determined during the consultation
